Eternal and incarnate Wisdom, Jesus Christ, true God and true man, I adore you profoundly. I thank you for having emptied yourself in assuming the condition of a slave to set me free from the cruel slavery of the evil one and for giving me yourself in the Most Blessed Sacrament.
I also praise and glorify you for having given me Mary as a Mother and Queen so that I may be your faithful friend, brother, and disciple through her. From this day forward, I turn to her intercession, mediation, and mercy. Through her and each of the Holy Angels, I hope to cease offending God and obtain from you true contrition, repentance, and pardon for all my sins.
“Woman, Behold your son” (Jn 19:26). Blessed Mother, living tabernacle of God, Ark of the new covenant, the new Eve, I greet you as Queen of heaven and earth since all under God is subject to your sovereignty. I seek a warm and genuine devotion to you and entrust myself and my apostolate to your maternal care and patronage. Help me obtain the virtues proper to my life mission, possess the fruits of the Holy Spirit, grow my friendship with your Divine Son Jesus, and always approach Him with me. In support of my petitions, please accept the promises and the offering I now make, despite my unworthiness.
Immaculate Conception: I renew and ratify my baptismal promises and confirmation commitments through you. I renounce Satan, his empty promises, and his evil designs, and I beg to be an instrument, along with each of the Holy Angels, in your ongoing mission to spread the Kingdom of God in the world. I give myself entirely to Jesus Christ, the incarnate Wisdom, to live a life of wisdom and virtue and to carry my cross after him for the rest of my life, to be more faithful to him than I have been till now and to the Most Holy Trinity through him. Today, with my guardian angel, St. Michael, and each of the Holy Angels as witnesses, without any reservations, I give you my whole life in time and eternity. I accept you, Mary, as my Blessed Mother and Queen. I surrender and entrust myself to you and your Immaculate Heart, body, and soul, with all my powers and all that I possess, both spiritual and material, including the value of any good works, past, present, and to come. My Mother, in whom I trust!
Accept, gracious Virgin, poor and humble when in the world, this offering of myself to honor and imitate your fiat at the Annunciation and at the foot of the Cross, as well as the dependence and obedience Jesus chose to have towards you, his Mother. I wish to acknowledge the authority you both have over me. I will honor and obey you as your true son and subject. O Blessed Mother, present me to your Divine Son as his brother, subject, and disciple now and forever so that he who redeemed me through you will receive me through you.
O Mary, teach, lead, and guide me; correct and enlighten my thoughts and behavior; defend me from the deceptions, distractions, ambushes, influences, and temptations of the Devil and grant me discernment; take over my entire life; and always keep me in your presence and in union with you. Help me live a genuinely Eucharistic life. Blessed Mother, make me one of those you support, nourish, protect, and love as your sons and subjects. Virgin most faithful, take away my inconstancy and haste. Make me, in everything, so committed a disciple of Jesus, your Son, Incarnate Wisdom that I may become worthy of heaven through your intercession, mediation, and example. Thank you. Amen.
